Client Intake Information

When you engage our services as a Client, you will complete a detailed intake questionnaire that collects professional biographical information including your career history, educational background, leadership philosophy, professional accomplishments, and other information you choose to share for the purpose of building your Leadership Profile. This information is used exclusively to produce your contracted deliverables and is never shared with any third party without your explicit written consent.

Index & Atlas may display completed Leadership Profiles as portfolio examples in our marketing materials, Website, and professional presentations to demonstrate the quality and scope of our services. We will not identify a Client by name in portfolio materials without their explicit written consent. Clients who prefer their profile not be used for portfolio purposes may request this exclusion in writing and we will honor that request.

We rely on reputable and secure third-party platforms including Squarespace for our website infrastructure and HoneyBook for our client management. These platforms maintain their own security standards and practices.
